98 Civic DX Automatic Transmission Problem?

I have a 98 Civic DX, automatic and recently I have been noticing what
appears to be strange shifting. For instance, when I roll up to an
intersection and the light turns green before I stop or when I make a slow
speed turn (into a driveway for instance) and then get back on the gas, the
transmission shifts down with a bit of a clunk (nothing too loud, just
abrupt/rough). Also, when coming off the highway onto a ramp (and on the
brakes) the car seems to be abrupt/rough when it downshifts (from 4th to
    3rd).

Does this sound like a fatal transmission problem or might it be an issue as
simple as new honda tranny fluid? Any suggestions? Thank you for your
    assistance.

when was ATF fluid changed out?
    check if fluild is low.
    might try changing out fluid.
If fluid looks bad, may need to change out fluid a few times, you can
drain only partial capacity of tranny, since some of the fluid in torque
converter can not be drained. you will have to run engine to flow new
fluid thru converter and rest of tranny.
Use the honda fluid from dealer.
Also Throttle Position Sensor may be bad, causing bad shifting.
