Cause of V6 Transmission Failures?

    To shorten a long story and quickly ask a few questions, My first
    Accord V6 transmission was replaced at 50k miles. This replacement
    was then replaced at 76k miles. The first trans had burned out 2nd
    gear clutch packs, and the second transmission seemed to have the same
    problem, but in 1st gear.

    I have been told by several sources that the trans design is more like
    an automatically shifted manual than a typical auto.

    My questions are these:

    1.) What technical explanations have people heard for the higher than
    acceptible failure rate?

    2.) What types of failures are others observing with their Accord V6
    and related (or identical) tranmissions such as the Odyssey, CL, or
    TL?

    3.) What was the rumored "fix" was that supposedly fixed the 02+
    transmission?

    4.) Are the newer 5 speed autos in the Pilot, Odyssey, Accord, CL, and
    TL any better than the 4 speeds they replaced, or did they just slap
    an extra gear in an essentially similar transmission?
