I know it's hard to diagnose this over the net but any opinions /
    experience is welcome.

I've got a low mileage 91 Accord (86000 miles) and lately I'm hearing
a louder hmmm type noise when moving forward. My wife thought at
first it was normal (she drives it daily) but I disagree. Best way to
describe it verbally is that it sounds louder than normal engine noise
but I only notice this when its in the range of 35 or 40 mph to about
65 mph. Above or below that I can't notice it. When the noise occurs
we can still talk and hear one and other inside the car. From inside
the car, the sound seems to be coming from the bottom front half of
    the car.

I went ahead and replaced all four tires after this noise began even
tho I know I've got a repair bill in front of me so the tires aren't
the cause. The sound doesn't seem to change based on road surface nor
which gear I drive it in. When I watch the tach vary, it doesn't seem
to change pitch. Only when the speed varies between the say 35 to 65
mph it changes pitch slightly. This seems to concur with when I step
on the brake it seems to change pitch slightly when in the 35 to 65
    mph speed range.

I will do a visual inspection after my back pain lessens but meanwhile
I'm trying to use the car and my brain to figure it out. I'm
beginning to think it might be the exhaust system (never has been
changed) but I wonder about the stepping on the brake as I described
above would rule it out. I will be taking it into Honda as soon as
time permits but I'd like to get an idea what I'm in for beforehand.

Like I said before, I know this type diagnosis is hard to do without
hearing it yourself but anyone have an idea what it could be??

It sounds like a brake wear indicator.

How about the heat shield on exhaust?

Everyone is suggesting noises that would be more of a rattle or a high
pitch squeel. I think you are describing more of a rumble. I would
suspect the transaxle. Honda has a known problem with the and has even
increased the factory warranty on all late model accords. I would have
the honda dealer check it out and tell him that the noise is from the
trans. Might be just a minor prob. right now and should be a warranty
    repair.... Good Luck

I doubt that his 91 accord is still covered under any warranty that would
cover the tranny. There was a TSB or maybe a recall on the tranny, some
nuts had a problem backing off.
